This is a title from WotLK, which requires the achievement Insane in the Membrane
It's a very nice, and rare title that requires:

Honored with Bloodsail Buccaneers(kill Boody Bay guards for that one)
Exalted with Everlook(questing in Winterspring)
Exalted with Ratchet(questing in the eastern Barrens)
Exalted with Ravenholdt(that's a place hided in the mountains in the southeast Alterac Mountains)
Exalted with Booty Bay(questing all the way around in Azeroth, check WoWhead)
Exalted with Gadgetzan(eventually do some quests, and rep framing close to Zul'Farrak)
Exalted with Darkmoon Faire(Darkmoon Faire is a event, which sometimes appears south for Goldshire, and right outside Thunder Bluff)
Exalted with Shen'dralar(to raise rep with those guys, do dailys inside Dire Maul, where they are)

As you may notice while trying to complete the achievement above in Cataclysm (Insane in the Membrane), many guides are outdated and updated guides are hard to find. So I came here to make a quick overview on the easiest ways I found to farm rep for the following:

Honored With Bloodsail Buccaneers: I would do this rep first, considering you might want to go back to the goblin cities afterward. (However, if you wish to get the exalted achievement also, I would recommend Steamwheedle first).

Before Cataclysm, exalted was the required reputation for this faction. To balance out the buffing up of guards in the cities, they have lowered it to honored. However, you still have to kill an extremely fun amount of guards to get to honored. Killing the guards in Booty Bay (With a few friends of course) is BY FAR the most efficient way to gain rep. A rough estimate of guard kills from full hated to honored is 2.5kish guards. Either way, it will be very hard considering they hit as hard as Heroic Cata dungeon trash and have just as much HP. They are also all over BB, so watch where you pull.

Exalted With The Steamwheedle Cartel: In for some pirate genocide? Better be. I started with this since I was already halfway into honored with all of them. Well, I hated it. Each pirate gives you 2.5 rep. With the 2nd rank guild perk, every 3rd kill is 3 rep, and for humans with the perk, every 2nd kill. Otherwise, you get 3 rep every 4 kills. The pirates are level 40-, so nothing to worry about if you already did Bloodsail.

There are 2 primary places I farmed pirates. There is NO way to get exalted without losing Bloodsail rep that I have found. Sorry. The first is Lost rigger cove and the pirate camp right below it. Those are East of the Caverns of Time in Tanaris, and the other is the Bloodsail ships and camps on the Wild Shore in Stranglethorn directly east of Booty Bay.

(Side note: I did EVERY camp and respawn on the shore, cleared the ships, waited 1 minutes for respawn, and repeated for Strangethorn route, and Tanaris I switched camps maybe every 5 minutes to keep the respawns up and the rep coming in fast)

Now, why can't you farm 1 camp the whole time? (Preferably the Southsea camps?) Well, there are 4 goblin factions. Everlook, Gadgetzan, Booty Bay, and Ratchet. Everlook and Ratchet you will get about halfway through, but Gadgetzan and Booty Bay are the problems. Once you hit revered, or more specifically 11999/12000 into honored with Booty Bay, you have to kill Southsea pirates to get exalted, and Bloodsail for Gadgetzan rep. It sucks and makes your life miserable. It took me over 40 hours of dedicated pirate killing. I killed over 30k pirates. This is NOT easy, but hey, 5/7 down, right? It's so close. It'll go by quick!

Wrong.

Exalted With Ravenholdt: My favorite, no idea why. Maybe it's because killing rogues tons of times is like a cruel act of vengeance for Warsong Gulch or something. Then again, Arathi Basin was worse, eh? Anyway, the 2 best places to farm it are the 2 closest areas: Stromgarde Keep and Northfold Manor (Or whatever the farm north of it is called). Syndicate is your target here. They are a band of rogues and mages, so have at it. I cleared the keep and went straight to the farm and back. Watch for stealthed highwayman (Keep) and prowlers (Farm) to kill. They give 5 rep per kill, which is a nice up from pirates.

Alas, that is only up to revered. Now you get to do something DIFFERENT! And annoying. Once you hit 11999/12000 honored, you will need approximately 1,400 Heavy Junkboxes. These can be farmed from various level 50+ mobs by pickpocketing. That's right! YOU ARE GOING TO NEED A ROGUE. OR TWO. OR ALOT MORE.

From what I've heard, the best place to farm these are in Blackrock Depths and/or Lower Blackrock Spire. There are a couple of glyphs and potions to help your stealth and pickpocketing abilities, so be sure to watch for those. You can hire people from trade, hop on your rogue alt and farm them, or enslave and brainwash your guildmates to do it for you! (Not like I did that >.>) And last but not least, do it yourself if you are a rogue! :D

These junkboxes CAN be lockpicked and looted of all but one item (Or it will disappear). Chance at a rare item or some lint or something. I'm not gonna bother to personally. These are turned into a guy in Ravenholdt Manor (Reached from going east from Tarren Mill in Hillsbrad Foothills, there is a dirt path like an S, then a cave where on the other side is a nice little castle.) who collects stuff like this.

Exalted with Darkmoon Faire: Well, truth be told, I have yet to do this. The reason being, this is the hardest rep to get if you don't take dumps of gold. The only way after 2000/6000 friendly (Which can be quickly earned by doing dailies/faire games when it's in town. Once you are done with dailies and games, you can do a turn-in quest involving thorium widgets to get to 2k.) to get rep is to turn in decks.

Epic decks = 350 rep each
Green/Blue decks = 25 rep each

Epic decks: These are from rare drops in dungeons. Best ones to get are probably from BC, considering they are the easiest to farm. However, they still go for a VERY large amount of gold to buy 106 of these decks. On my server, Nesingwary (US), they went for 1-2k. That's 100-200k gold for exalted. Sorry, nuhuh.

Green/Blue decks: These can be crafted by scribes (Or inscriptionators :P). The mats cost on Nesingwary (US) were about 50g per deck. I did the math and the least expensive was the Rogues Deck. I would recommend farming the herbs, but if you have the money, be my guest to buy it off of the AH and make the decks.

I did the math and if you bough the mats for the costs listed, it would be over 100k gold no matter what way you went. So, your decision if you want to farm or buy. I am stumped on what I am going to do, but I am leaning to buy. Now to get the money. Hopefully with Cata, there is a farming paradise up ahead for me to make money for this off of.

I completed this achievement about a year ago, so take that into consideration, however I'll add my two cents.

I found the best place to get goblin rep was believe it or not, Dire Maul North. I don't know why they removed Shen'dralar from the list of reputations, as its actually not nearly a tenth as bad as Darkmoon. I would clear out dm north while using a friend's char on follow, pick up as many books as I could while searching for a shackle key, and putting doubles of books and keys on the other character. The rep from turning in the shackle quest and ogre suit 5 times an hour far exceeded any grind in tanaris or north barrens, not to mention was far less mind numbing.

edit: The thing to note here is that having a friend account on follow was absolutely key here. The ability to trade keys and books made it twice as fast. Now, given that we can fly at 310% speed, I would have to recommend you just lay siege on pirates in tanaris.

Ravenholdt should be done with a rogue at level 58 with subtlty and appropriate glyphs to make it fast and efficient. Shadow step is very useful. Leveling a char to 58 should be nothing to you if you plan on doing this achievement tbqh. I found LBRS is the only place you should search for boxes. The pockets of these mobs actually respawned by the time i made it to the top, so you dont need to reset. Roll a goblin for a pack hobgoblin to stash these as you will need thousands.

Darkmoon cost me around 30k to finish. I was able to snag alot of berserker decks for 200g. This may no longer be an option anymore, since nobody builds northrend decks anymore, however northrend decks will be far easier to craft than outland or cata decks. Level a dk (for flight speed) or a tauren druid (fast herbing) and herb in northrend.

In the cataclysm expansion however, I would suggest you stick with the low level inscription decks that give 25 rep a piece. When I got this achievement, I found the low level decks to be a waste of time, however with a tauren druid with herbalism and 310% flight speed, you could potentially crank out hundreds of the low level decks a day. This may actually be your best or more likely, only feasible option since nobody builds wotlk decks anymore. Buy any decks you can afford and herb like mad.

For those working on Ravenholdt: The best and fastest place to get Heavy Junkboxes is BRD. Your rogue only needs to be lvl 55

Get to the entrance to Blackrock Depths, go left through the shadowforge gate, right through the next shadowforge gate, then left through the East Garrison door with the friendly NPC standing in front of it. This will take you into the East Garrison. Go up the stairs, stealthed of course, and be sure to avoid the Fireguards (they have an AoE fire aura that will screw your stealth). You can easily get here at lvl 55, and all the NPCs here are lvl 50 - 53 (with two or three 55s sprinkled in). This will put you in the Ring of Law, among the spectators on the upper level.

Its a big circle, with groups of 9 - 12 NPCs in intervals all around it. Just circle it over and over again picking all the pockets, with detours into the East Garrison (containing about 15 NPCs) and Shrine of Thaurissan (containing about 18 NPCs). Doing this, by time you do a full lap and come back to your start point, all the pockets are pretty much full again. Worst case scenario, there is one dog in the shrine that can detect stealth. If he's there, obviously, avoid that group. Otherwise, there is no stealth detectors at all.

I've been doing this myself after beating my head against the keyboard trying to find a good post-cata spot to farm these boxes. My rogue is lvl 56, and I was using LBRS, but heard about a few peeps having good results in the Ring of Law, and gave it a try. LBRS got me about 20 - 30 boxes per hour, but BRD in the Ring of Law has gotten me 80 - 90 boxes per hour easily. I strongly recommend this place if you want to get the Ravenholdt rep grind done faster.

Don't know how to get to BRD? Inside Blackrock mountain, walk across one of the huge chains to get to the giant rock out in the middle. Follow the pathway to the bottom, cross another chain, and talk to the NPC with the quest (Lothos Riftwaker). You must accept the quest to be able to enter BRD! After getting the quest, take the doorway there next to Lothos, and go down into the big chamber at the bottom (the Grinding Quarry). Bear left, all the way into the Masonry (from a distance, looks like a red-glowing room), all the way to the back in the corner to the right. If you die inside the instance, don't worry: the graveyard is right outside the instance door.

sorry for being a bit rube but for the Gobbies u idiots farming pirates from hated or unfriendly (if u done the pirates first) hand in the cloth, from bootybay, outside the sharkmouth go out then turn right its a girl pirate on a bit of a hill u hand in silk, done u get all to friendly and honored BB then go to everlook and hand in the other cloth u will get honored with everlook and a bit with bootybay, then go to gadetzan u will get honored with that and a bit with the others then go to ratchet and i dont remember that one but i know if u hand in all cloth u get exulted, i did this in BC before the achevement builds and well they came out as i had a pirate suit and i was like woo wooo i got 5 reps / 8 then get the shendralar and that was easy the other two (DMF and Ravenhold) are just a pain and take lot of effort and motorvation i suggest doing them ones at low level and then grind up the other 5 and u will get heaps

This is all just not true. The requirement for Bloodsail Buccaneers rep has never been higher than honored, since before Cataclysm, the only way to advance your rep past 11999/12000 honored was to kill the same low-spawn Goblin NPC in the Badlands some x-thousand times (and the three quests, if you saved them till this point).

Additionally, the guards in Booty Bay simply hit too hard and have too much HP for any person trying to solo their rep. The only reliable way for a solo person to advance their Bloodsail rep in Cata is by killing the Goblin quest givers in Faldir's Cove, Arathi Highlands who award 5 rep each and have a short respawn time (since they are quest givers). That is, until we get a few more raid tiers into Cataclysm and people start becoming able to solo the guards themselves.

If you are looking to get the Bloodsail Bucaneers rep and can not get a group, there is a spot called Faldir's Cove which has an entrance at in Arathi Highlands. The people in this area are all Booty Bay, all can be killed but 2. The regular 'booty bay people' spawn quick and yield 5 bloodsail rep per kill. The 'named' NPC's give about 25 rep each...

It is long and slow, but an alternative for if you can not get a group. This is how I got my pirate rep.

I finally finished this achievement on June 9, 2011! A few tips that may be helpful for some folks:

Ravenholdt - For farming the Heavy Junkbox, I found LBRS to be the easiest location (looted several Sturdy Junkbox in BRD, which is not the right kind, so I left there). Start on the mobs near the entrance, but then drop down to the bottom before you cross the first rope bridge. Then work your way back up to the top, rinse and repeat. By the time you get back to where you started, the mobs' pickpocket loot will have respawned, so there's virtually no downtime. If you have the engineering portable mailbox or the Argent Squire/Gruntling, you can mail the junkboxes to an alt in order to clear your bags without even leaving the dungeon, which further reduces downtime. If you don't have a portable mail option, simply set your hearthstone to the nearest inn (which is in New Kargath, I think).

This is the mouseover pickpocket macro I used, in case you don't have one:

As a side note, there is some profit to be had here. I ended up looting about 50 Star Ruby, many from regular pickpocket loot, many from the junkboxes themselves. (Yes, I looted every junkbox, making sure to leave at least one item to prevent despawn. If you plan to loot the junkboxes, be sure to turn off autoloot or hold down the shift key to override it.)

Darkmoon - It isn't actually necessary to make or buy all the Darkmoon decks in order to reach exalted. A far better method is to make or buy several of each deck, and then turn them in when the fair is in town. Now you have several of each trinket in stock, and you can start advertising in trade - "WTT Darkmoon trinket for your deck, PST!" or something along those lines. I added a 150g bonus payout as extra incentive, although many people will take you up on the offer just to save themselves a trip to the fairgrounds. The key is to have the trinket on hand so that you can make a direct trade, which prevents "OMG, SCAM!" responses. While the fair is in town, you can do this all day long! Simply return to the fair and turn in your decks in order to replenish your supply when you run out of trinkets to trade. Be sure to turn in all your decks before the fair leaves, which will allow you to continue trading after the fair is gone, up to the point where you run out of trinkets. Of the 60 or so decks I've turned in so far, over 40 of them were decks that I didn't have to make or purchase myself, which has saved me tons of time and gold.

Another side note: for those of you planning on making your own decks, it probably goes without saying that a druid herbalist with Master Riding is hands-down the best toon to use for farming Whiptail in Uldum, which is by far the easiest source of herbs to use for making cards. With it takes me about 3.5 hours of farming in Uldum to gather 2k whiptail (100 stacks), which in turn requires ~51 minutes of nonstop milling and ink-making, which in turn yields about 16 cards. Assuming best-case scenario (no duplicate cards, or you're able to trade duplicates instantly with other players), you can make 2 decks every 4.5 hours. Assuming you benefit from Mr. Popularity, you'll gain 385 rep per deck turn-in, which means you'll need 97 decks total to reach exalted from 2k friendly. If you make all 97 decks yourself, that adds up to ~218 combined hours (over 9 days) of /played time, doing nothing but herb gathering, milling, ink-making, and card-making. Grats if you have this much dedication; I chose the easier route. =P

Steamwheedle Cartel - Find other players on your realm to group with while slaughtering pirates in Tanaris. Not only do you gain rep from kills by party members (as long as you're within reasonably close proximity to each other), you also benefit from faster mob respawns. Apparently, the faster the mobs die in that area, the faster they respawn as the game tries to maintain a minimum number of them at all times. That may not be the most accurate way of describing the respawn mechanic for that area, but it seems to be the case just from my own observations. Just by grouping with one other player last night, I gained 5k rep in about 1.5 hours. Another option, which also involves grouping with other player(s), is to have one party member do all the killing for an hour or so, while the other party member(s) auto-follow him. The other players can tab out of the game to surf the net, watch a movie, take a nap, or whatever, while gaining rep the whole time. Then the first guy can take a break while someone else takes over.

Edit: By the way, it seems that you can now reach 20,999/21,000 revered with Gadgetzan and 999/999 exalted with Ratchet, Everlook, and Booty Bay without having to leave Tanaris. Before, once you reached 11,999/12,000 honored with Gadgetzan, you stopped gaining Gadgetzan rep from killing pirates in Tanaris. At that point, you had to go kill pirates in the Barrens or Venture Co mobs in Stranglethorn in order to finish your Gadgetzan rep. I don't know when this was changed, but I confirmed it in the week leading up to my Insane achievement/title.

Not sure if this has been posted yet, but I didn't see it. When you're trying to get your Steamwheedle Cartel rep back, just Southweast of Ratchet there is an island named Fray Island. I went there with a friend one night, grouped up with someone else that showed up and within 3-4 hours we went from Friendly to almost Revered. I've gone there by myself since and it's still super fast respawns and the mobs have really low HP. There's two buildings and a ship, I usually don't even bother with the ship because the rest of the mobs on the island respawn so fast.

Anyone who mentions wanting to get their Steamwheedle rep back up, first thing I do is to tell them to go there.

It kind of sucks to get there when hated because the Ratchet mobs will kill you, so get a port to Theramore and fly up. If you're horde: go to Thunder Bluff and fly over.

Rep Rates in Diremaul:
Even if you get 5 keys per hour in DM + 10 Tannins (1 from knot, 1 from basket), the theoretical absolute maximum rep/hour is 5*350 + 10*75 = 2500. My own rates (based on 4 separate 1-hour tests) were closer to 1150 rep/hour (2 keys + 6 tannins) for 5 full "miniboss clears" per hour. My luck absolutely STINKS though, so I'd be willing to accept that you can average up to 2000 rep/hour in DM.

Rep rates outside Ratchet:
Nothing complex here: I have been able to maintain well over 2500 rep/hour without too much effort simply running around killing southsea freebooters (and Baron Longshore). This is 2500/hour Everlook/Booty Bay/Gadget "spillover" rep -- actually 5k/hour Ratchet rep, and I have read that 3k-3.5k/hour spillover is possible.

Rep rates in Tanaris:
Have only done limited testing here, but was able to maintain 2100 rep/hour grinding wastewander (while half asleep) and I am guessing a good deal more from the southsea pirates. Again this is 2100/hour Everlook/Booty Bay/Ratchet "spillover" rep -- actually 4.2k/hour Ratchet rep.

The point is, even the half-credit "spillover" rep is faster grinding pirates than what you can get in DM.

Getting to exalted:
Most people assume that because the direct rep from pirates stops at revered, that you *must* go to DM (or zap giants) to get to exalted. This is untrue. The key is to switch zones at different points in your grind to make sure you are always progressing:
1. grind one area at full-credit until you hit revered (and stop gaining rep with the local faction),
2. switch to the other zone and grind until you hit exalted with the originally capped faction
3. switch back to original zone to finish grinding the other 3 factions to exalted.

Example (which I am using personally, though only about 30% through step 2a.):
1a. Started grinding south of ratchet, from 0/36000 hated for all 4 factions
1b. Grind Ratchet rep to 11999/12000 (revered), resulting in the 3 other factions at 31500/36000 hated
2a. Switch to Tanaris, and grind until Ratchet is exalted (another 21000 rep), resulting in:
Everlook & Booty Bay at 1500/12000
Gadgetzan at 11999/12000
3a. Switch again to pirates south of ratchet, until all factions are exalted

The downside is that you don't earn any cash (the rep rates assume no looting), but it is definitely faster than the DM key grind, even if your luck is considerably better than mine.

I don't know if someone mentioned it already, but another good Cartel rep place in Venture Bay in Grizzly Hills. The Venture Co. mobs give Cartel rep without losing Bloodsail, just like the Venture Co. that was in Stranglethorn (there are still a few mobs there but not as many as pre-Cata).

I've just completed this achievement 30 minutes ago and realized two things that I had not taken into full consideration in regards to Darkmoon Faire Rep and Malkaris33's post.

I chose to farm Icecaps in Winterspring and powered out the entirety of this achievement in three weeks. Having completed it... I now have 120 Darkmoon Cards that are not capable of being held in the same bag or bank space and must be stored in my mailbox and that are fairly valueless.

Taking the extra week to have farmed Cataclysm herbs, even at an approximate third of the end amount of herbs necessary to herb would have been, I would ended up with items with an end value of greater than 400,000 gold and the ability to all be held within a singular bank and inventory space. Take the time, get the achieve, make the money, have an easy next month of juggling crap in your inventory.

There are changes to this achiev since cata, not only removing some requirements but Blizzard has increased the difficulty "some" to compensate for the other factions gone, made changes to NPCs etc etc. Please check your sources before posting/doing. I went out verified everything I am writing now with the current patch.

DMF for this acheiv just takes ALOT of grinding herbs or dropping some cash. I did a little of both. Paying people to turn in their decks can be cheaper then making or buying the epic decks yourself. I just advertise on trade. Also, I don't find the cards valueless, I have alot of people buy the 60s or 70s cards for 150-300g each. Not a killing but good to make a little money back. Post 4.3 you can still turn in the card decks for rep.

Ravenholdt is pretty easy as well just grinding out the rep to honored 11999/12000. The Northfold Manor, and Stromguarde Keep are the places for those. Then either farming the heavy junkboxes or paying for some on trade. Outside of Blackrock mountain, there are non elite NPCs that are all bunched up in 4x4 groups that are easily pickpocketed. Kill them and they respawn for you, then repeat.

Bloodsail, however, is a big pita, since cata. The guards are damn hard kill to solo, you can die very easily. (esp if you are a clothie like me) Unless you have a 5 man raid with tank and heals. The NPCs in arathi highlands NO LONGER EXIST to give you the easy farmable, if slow rep. The only people there now are a couple of friendly NPCs.

however, you can grind rep during a questline in booty bay the bruisers will drop down to level 35. the Bruisers don't give rep but the NPCs do, like shop keepers etc. This was a guy's post. I pasted it here for easy access. (not my post)

If you have not done any of the Stranglethorn quests prior to Cataclysm, or were partially done and had the whole zone reset on you (as in my case), you can use the following guide to have really easy grind.

1. Do some quests. No, really.
- Talk to Sea Wolf MacKinley first, and do his two quests - Scaring Shaky and Kill-Collect
- This will unlock The Bloodsail Buccaneers from First Mate Crazz. As you can see on that quest's page, there's a whole chain of quests to go through while you rub elbows with the Bloodsails at the bidding of Baron Revilgaz. Sadly, you do not earn Bloodsail rep for doing their quests.
- At the end of the quest chain, you'll be sent to your very own Captain's chambers aboard your own pirate ship (woo!), where you find (Item #204450). This begins a third, smaller, quest chain in which you sabotage the Bloodsails, learn about the Brashtide Crew, and head on back to Booty Bay to report your findings.
- The last quest you need to do is Return to Revilgaz, because phasing kicks in, and everyone in Booty Bay is dropped down to level 35.

2. Set your status with Booty Bay to "At War."

3. Have fun killing things!
- Everything is now easily killable, but be warned - the Bruisers do NOT give you rep. You must kill other, named NPCs around Booty Bay for reputation. The only Cartel-affiliated NPC you cannot kill is Myizz Luckycatch the fishing trainer, who is now amusingly enough, dangling from the rigging outside the Inn.
- Some of the NPCs will give you 25-28 rep per kill - Baron Revilgaz, the two bankers (Rickle and Viznik Goldgrubber), and Grizzlowe the shipmaster. Wharfmaster Lozgil will give you 15-16 rep, and pretty much everyone else will give you 5 or 6 rep per kill (including Fleetmaster Seahorn).
- Two paths that I took to do this grind:
Route 1: Start at the end of the dock where Grizzlowe is cowering. Make your way up the dock, and head into the Black Market building. Once you've dealt with everyone inside, come back out, mount up, and fly up over the building to the main entrance to Booty Bay. There's a path up there with a couple buildings; kill the 5 or 6 guys standing out front, and Dizzy One-Eye, who is across the first bridge. Now, usually I'd just keep going and hit up all of the profession shops along that top path until I got to the Alliance flight point, and then jump down (by the hanging shark). It drops me right into...
Route 2: It doesn't really matter where you start, but if we're starting #2 at the end of #1, then this is the route you'll take. Kill Jutak, then head into the inn. Kill the 2 goblins and the gnome drinking right inside the door. Run all the way upstairs, killing Catelyn and Deeg along the way. Don't worry about those two rooms across the bridge-thing from Catelyn; no one's in them. Now you're up top where the Baron is hopping around yelling like a madman. Kill him, then take the little side path around the corner to the Horde flight point. Kill Kebak and the Bruiser, then hop down into the bank area. Kill both bankers and Privateer Bloads. Run up the ramp back to the shark, and take care of Seahorn, Sea Wolf, and the sick goblin lying in the little alcove. You won't be able to tab-target him, but he is killable for rep.
Route 2 is the one I usually do. More or less, by the time you make a full lap and get back to an NPC, it will have respawned. Jutak and one of the drinking goblins have longer respawn timers, so you won't get them on every lap. The bankers occasionally take a little longer to spawn. If you do Route 2 a lot and find that the NPCs aren't spawning fast enough for your liking, fly across the harbor and do a run of Route 1.

4. The absolute best part about this phased quest?
You can pick up and complete Avast Ye, Admiral!, earn 550 Bloodsail rep for it, and then keep going! At the moment, since it's such an easy grind, I'm going to see how far it will take me, or if it will cap at some point.

Venture bay in northrend and Fray's Island and Venture Co base camp in N. Stranglethorn are some places to grind back your goblin rep. If you want to save your bloodsail rep. Its easist if you don't care about keeping your bloodsail rep but I do so, I don't intend to do any questing. If you don't care, go quest or slaughter endless amounts of bloodsail pirates for some rep.

And FYI, you only to have the bloodsail rep reach honored, for the Insane Title...but I would recommend going all the way for achievement safety issues and to get counted for another exalted rep.

I hope this helps to clear up alot of confusing patch changing comments.

If you're grinding rep in Tanaris by killing pirates, it's assumed you'll hit neutral with Gadgetzan before the others, since they give 5-6 rep for Gadgetzan per kill.

Once you hit neutral, as of Cataclysm, you can go to Gadgetzan and do the quests there without losing bloodsail rep. This is what I did. There's also 2 or 3 quests in Fuselight out in the Badlands you can do for a good chunk of rep too, also without losing Bloodsail rep.

This is what I did, saved me a few thousand pirate kills.

However, do not do the repeatable quest to turn in water. I didn't try this, and it may cause you to lose reputation.

Come 4.3 there is going to be new quests for DMF rep.
750 from PvP quests
2250 from General Quests
And 3750 from Profession quests.
Keep in mind MOST people will only get 1500 reputation from the profession quests as they dont have all the professions. But the profession quests do NOT have a level requirement so you could drop professions and learn a new one to get the quest. (Its cheaper/less time consuming then farming a deck). Trust me. It has been confirmed that the profession quests repeated - "up to five skill points per profession, per Faire week, if you’re good at what you do!"
But it hasnt been stated if they will give rep a second time.

All reputation numbers here are assuming you have no rep gain bonus.

Latest Changes ( Patch 6.0.3 - WoD ) :

Insane in the membrane is still available on patch 6.0.3 . It doesn't matter if your exalted first with steamwheedle then your getting honored with bloodsail or the other way around as you just need to trigger the minimum requirements of this achievement .

To confirm that , This is a small macro that will put this achievement on track :

Everytime you complete one of the requirements for this achievement , it will be triggered .

Bloodsail Buccaneers :
The guards in bootybay has been buffed , they are elites and are at level 100 . Hard to solo by one player i assume so you may need a group. Killing a guard will grant you 27 rep with Bloodsail Buccaneers . The alternative solution to gain reputation is to kill the npc's ( 5 npc's total ) at Faldir's Cove in Arathi highlands . One of the npc's called shakes o'breen will grant you 5-6 reputation per kill and his respawn time is 10 - 15 seconds . In theory you gain 22 rep / min , Around 1320 rep per hour . Note that this is a slow way to gain rep compared to killing guards at booybay but it might be your only option to gain reputation all the way to honored with bloodsail . One more thing to mention that the maximum reputation you can gain by killing npc's at Faldir's cove is 11999/12000 . If your aiming for exalted then you need to farm bootybay .

Steamwheedle :
If you want to gain reputation with steamwheedle , in addition to killing pirates at tanaris / ratchet and in addition to completing some goblin quests , There is an npc north west gadgetzan ( 50,27 ) called Rumsen Fezzlebrack . A repeatable quest called War at sea will be available Only if your hated / hostile / unfriendly with steamwheedle . Completing this quest will grant you 500 Rep with Gadgetzan and 270 rep with the rest of the goblins cities . In addition you will lose 500 rep with bloodsail buccaneers .

Note : if you want to be exalted with bloodsail and steamwheedle in the same time , your only way would be to farm bloodsail rep at the start , all the way to exalted , then farm steamwheedle rep by killing the pirates in tanaris / ratchet .

There is a small change regarding Ravenholdt faction since 4.3:

The person to turn in Heavy Junkbox (Quest link: Junkboxes Needed) has changed. It used to be Fahrad standing on balcony of Ravenholdt manor, but now changed to Winstone Wolfe inside ground floor of the manor. This means different thing to different players:

For those engineers with Jeeves and MOLL-E (or with engineer alts / friends), no difference for you.
For non-engineers who have something like Reins of the Traveler's Tundra Mammoth for selling Dusksteel Throwing Knife, bad news for you. Since new NPC is indoors, you can't use your valuable mount anymore.
For those who don't even have such mount, good news for you, because you don't need to run between floors to sell Dusksteel Throwing Knife (as vendor is located on ground floor too).

I wanted to get clarification on some conflicting info I was reading on forums here and otherwise, about maintaining rep with Bloodsail while gaining rep with the Steamweedle cities. It seems there wasn't a clear distinction on whether or not you needed to have BOTH at their required levels at the same time, to nail the title, so I opened a GM ticket...

You must maintain your honored, while rebuilding with the goblins. Believe it or not there are ways to do this, however they aren't easy. In fact, one might be....insane....to attempt it. (See what I did there?) There are many walkthroughs posted online however, and I have faith you're just the insane sort of person to succeed where many have failed!

So this means that you need to have Honored with Bloodsail, WHILE getting to exhalted with the goblin cities. The very slow painful option of the quests and limited mob farming is the only one now then I guess?

Has anyone completed this achievement since 4.3 dropped (or even 4.x), while NOT having honored status with BS at the same time as exhalted status with SW?

Having done all the requirements for the achievement on 2/11/12, I can confirm you DO need to have Bloodsail at honored at the same time as having the goblin reps at exalted. I grinded Bloodsail to honored, then tanked it to get the Goblin reps to exalted; having all the other requirements filled, I did not get the achievement.

I'm sorry, but the whole "Insane" became a joke. It's just easy now and doesn't take a lot of time.

1) Started with Bloodsails - killed over and over 1 mob (Faldir's Cove, near the ship).
2) Then Goblins fractions - again 1 mob in a hut (Fray Island), then kill other mobs (use banner).
3) Darkmoon Faire - over 10k rep per month it's so easy now to get. You can turn cards as well.
4) Ravenholdt - that's the last I need actually. You can buy boxes or... Here are some simple tips:

- use glyph for pick pocketing (and opening boxes if you willing to check them)
- talents for faster moving while in Vanish (Nightstalker) are nice
- Minor Speed enchant for boots also helps a bit
- use "/targetenemy /cast pick pocket" (I have it assignet to a mouse wheel)
- I'm using LBRS, after clearing it I'm making a tea and waiting a bit for loot to respawn
- when bags are full I use heartstone to Iron Summit (Alliance) and send boxes (after checking)
- main has mailbox + "pet" with a mail; received knives are disenchanted and mats go on AH

To those hoping to get this achievement:

This achievement and title are personal. People will not stop and whisper you, "Woah, dude, you got the Insane? Nice!" often, if at all. If shooting for this achievement, do so only so you may show yourself the dedication you have to killing lots and lots of things. If you try get this achievement to get noticed by others, you will be very disappointed.

That being said, finally getting this achievement after all of the grinding is an adrenaline rush of all its own. I am never going to take this title off of my mage, that is for sure.

If you are farming Bloodsail rep using the phased quest method mentioned, I've found this method to be the most efficient as the mobs respawn rates are different. Using the method I mention, it is possible to get up to 1000 rep every ten minutes of farming.

1. Hit the trainers. They are worth 40 rep.
2. Fly down to the marketplace. Killing Crazz on the roof and the people inside nets you 40 rep.
3. Then head up to the upper levels where One-Eye and his friends are. Killing everyone there gives 40 rep.
4. Finally, make your way through the lower levels, killing the mobs inside the houses and the mobs hiding behind the backside of buildings. They are worth on average around 15-25 rep, depending on how much time you waste going inside and outside of buildings.

Once you are done with those, it's time to do some farming. The rest of the mobs all have respawn rates from 1-2 minutes.
5. Farm the tavern, which every minute gives you 15 rep.
6. Farm the auction house, which every minute (or 2) gives you 55 rep.
7. Farm the Baron's ledge, which nets you 35 rep every minute. But be careful if you are alliance not to AoE Kebok, or you will aggro the Horde flightmaster who summons a couple of adds and chases you around town.
8. Farm Seahorn's platform, which every minute gives you around 15 rep.
9. Finally, farm the dock, which every minute nets you around 20 rep.

There are additional mobs in the farming areas which may give you extra rep, but have longer respawn rates. I haven't mentioned them here just because it makes the process more complicated.

Additionally, the auctioneer outside of the marketplace doesn't give rep when killed, so don't bother. I have missed a few mobs who hide out in buildings or around corners. They might also give rep, but you can often mount and fly to another area faster than it would take to search for them or run up or down stairs to reach them.

Once 10 minutes have passed from your initial kill in the above rotation, head back to the trainers. Rinse and repeat. Should net you around 1000 rep every ten minutes. Doing this with a friend will likely speed up the process immeasurably, as you can just camp at the farming sites instead of having to fly around Booty Bay like a crazy person. The auction house is the best place to farm, so always check back to see if the auctioneers and bankers have respawned.

I have done a good deal of the goblin quests, is it still possible for me to get this feat and the title? I'm not sure, since the only way I know of to get goblin rep without detracting from pirate rep is these quests. Please, someone let me know so I can start the long grind / give up until I lvl an alt. Thanks. :)

I had a chat with the GM, and it seems that you don't need to keep honored status with bloodsail. So aslong as you reached honored with them once then you do not have to it again, and ruining the rep with them doesn't matter. It still triggers the achievement, so if you gain reputation with any of the other factions and it ruins the rep with bloodsail then it doesn't matter. Once you have got it, and you lose it. You still have it. Can anyone confirm this?

Just wanted to point out that since thrown weapons were removed with patch 5.0.4, the junkbox turn-ins now reward REP ONLY. Nothing else.

A word of warning as of 28/01/2013

I have completed all criteria with exception of BSB to honored and thought I had nothing more than to knock it off yesterday, so off to Faldir's Cove I went and in a very short time I noticed I was no longer Exalted with Booty Bay, shortly followed by the other three, to be expected obviously, but I also noticed that on the achievement tracker, these items were still checked off so I assumed I was home straight, grind through the night and I would be Insane come morning. Note, at this point I was also 29/30 for the achievement for 30 exalted reputations.

Anyway tiredness set in, so I logged out for the night making for an early start in the morning.

When I logged in, I noticed that the achievement tracker was now showing that I needed the 4 goblin factions exalted and also my 29/30 was now 25/30 I became very, very concerned, but I had no choice but to continue, I was hated with Booty Bay and not so far away from being the same with the other 3.

Anyway, I continued my grind until I finally hit Honored with BSB and guess what, no achievement and no title.
My achievement tracker now shows that I still need exalted with the 4 goblin factions.

Now this is the interesting part, I planned ahead, I asked 2 different GM's if this was possible to do the achievement in this way and was of course told that it was, I've retained the tickets of them stating this and to further back up my claim, I took screen shots along the way.

I've contacted a GM and await their reply, but I'm not holding out much hope or holding my breath, I think this will be one to have to simply chalk down to experience but this is game breaking for me, I spent a lot of time (don't have that much time to play the game anymore) and effort into getting this achievement and followed the direction of not one but 2 GM's and I really do think if this is not resolved pretty quickly in my favour, I'll be taking a break from the game, something I've not done in the 7 years or so I've played.

My final thought and advice, if anyone is contemplating doing as I did above, I would suggest not logging out during the BSB rep grind as the achievement tracker only changed after I logged out and back into the game.

I'll update on receiving correspondance from a GM

UPDATED: GM fixed for me, FOS and title across my chars.

I haven't read all comments since I'm such a slow reader. But if no one has written it yet, I would tell you that there is a place where you can farm Steamwheedle rep without losing your Buccaneer rep. It's on a ship in Venture Bay.
Mobs gain you:

+5 reputation with Booty Bay
+3 reputation with Everlook
+3 reputation with Gadgetzan
+3 reputation with Ratchet

NOTE: IT DOESN'T DECREASE REP WITH BUCCANEERS.

You can do it very fast nowdays since there's no spawn time, which means you can go there 2 people and just stand in 1 spot and do damage just by tapping and pressing any ability/spell. The mobs are level 73 and have around 9k hp.

In the "phased version" note that some NPCs are grouped together... McGavan, Brikk Keencraft, Jensen Underwood, & Hurklor (grp 1) are up the ramp from where the boat brings you into Booty Bay, and Dizzy One-Eye is very close to them as well. Another group are in the first big building to your left... Narkk, Kelsey Yance, Kizz Bluntstrike, Markel Smythe, Haren Kanmae, Mazk Snipeshot, and Zarena Cromwind (grp 2). Catching these groups (with an AoE) when they are all available helps with that grind when you are waiting for the big rep givers to respawn. I alternated between the big reppers with the groups of small reppers (mainly due to the fact that I was not the only one killing Booty Bay NPCs at the time...) Be aware in the "phased version", Booty Bay Bruisers do not give rep... Lastly, if you accidentally kill a "Brashtide" Pirate (because they are in your way, or they are annoyingly attacking you)...you will not lose Bloodsail Rep.

* The "Phased Version" of the Rep Grind is for those who have not done any Booty Bay quests (the main reason people do this is that the Booty Bay Bruisers will drop from Elite 100s to non-elite 35s)... You do a few quests for "Sea Wolf" MacKinley and Baron Revilgaz, which will eventually lead you to doing some quests for Fleet Master Firallon (which sadly does NOT give Bloodsail Rep). When you report back to Baron Revilgaz, he will have you fetch some Doublerum from his room. As you "tap the keg" your screen will "shake" and the Bloodsail are now attacking Booty Bay in a phased event. Set your Booty Bay rep to "At War" and can then start attacking Booty Bay NPCs

When turning in all og your Heavy Junkbox's for Ravenholdt rep, you need a mailbox. If you dont have Engineering, you can now get GUMM-E, its almost the same as MOLL-E but can be used by even if you dont have Engineering!.

Just a heads up: if you are on this grind and you see someone about to hand in a quest, please have some consideration and allow them to do so before you snipe the NPC. I remember trying to do the questlines in the phased area a couple years ago, and it wasn't fun to be stuck there for an hour just coz some asshat !@#$%^& was sniping everyone from the shadows.

Bare in mind, for me this was an alt so I got bored and swapped to another character. But new players are totally turned off from the game by this kind of behaviour. Just show a little restraint, and let them finish their quests so they can move on.

Nothing much changed in BfA, even with account - wide reputation. You still have to achieve all required Exalted reputations on one character.

So, I did the Bloodsail thing back in 2015, and am only now picking up the achievement again. I already did the cloth turn-in to become neutral with steamwheedle. Though now I'm wondering if I actually got all the way to honored. I'm FAIRLY sure I did, but again find myself wondering.

Is there any way to check if you meet this requirement and got Bloodsail to honored?
