You can't clear it ingame.
You have to exit WoW, open your WoW folder and delete the Cache folder.

If you, like me, have installed WoW under the default location you want the bat file like this:
@del "C:\Program Files\World of Warcraft\Cache\WDB\enGB\creaturecache.wdb"
@del "C:\Program Files\World of Warcraft\Cache\WDB\enGB\npccache.wdb"
@start C:\"Program Files"\"World of Warcraft"\Launcher.exe

I don't think you should have any problems if you follow the advise given by the addon's creator:
Advanced users can delete “C:\Program Files\World of Warcraft\Cache\WDB\enUS\creaturecache.wdb” specifically to improve log-in times, where enUS is your language code.
Of course, the path doesn't necessarily have to be C:\Program Files\...., but if anyone was inspired enough to put WoW in a different place, then I guess they know where they put it anyway. :P
I tried all of this including deleting my entire cache folder, then i did this step looking for the specific file that its cached into and i dont seem to have the file, yet deleting the entire folder doesnt clear my npcscan at all... please help

Make sure, in the NPCscan options menu in-game, that you have the 'Search for completed Achievement NPCs' box checked (it's right above the list of NPCs). Though it shouldn't affect newer rares, the WotLK and BC ones are certainly included in that, and if you do not have it checked then it will ignore them if you've killed them in the past.

You can put relative paths in the batch file, for example if the batch file is in the WOW folder (clearcache.bat)
@echo off
del Cache\WDB\enGB\creaturecache.wdb
start Launcher.exe
